OGE Form 201 (Request an Individual’s Ethics Documents) reapproved by OMB

The OGE Form 201 (Request an Individual’s Ethics Documents) (Form) was reapproved under the Paperwork Reduction Act (PRA) by the Office of Management and Budget.

The new expiration date is 5/31/29.

OGE made several changes to the Form with the goals of updating citations for accuracy, streamlining the Form to improve the user experience, and reduce the burden on requesters utilizing the Form.

OGE maintains two versions of the Form, a PDF version, and the online version. The online version enables the requester to electronically fill out, submit, and receive copies of the public financial disclosure reports available from OGE.

An updated copy of the PDF version has been posted to OGE’s website in the forms library. This is the form you can use to request ethics document from one of the 140 agencies across the executive branch.
